Designate who can access your account

Your health and benefit account provides you with secure access to your information and funds, but you may have situations when you may need someone else to get account information and/or make transactions on your behalf. Designating an authorized representative(s) for your account allows access only to those you name.

There are two types of authorizations you can select for your account:

Authorization to Act/Release Account Information

You can designate an individual to receive information about your health and benefit accounts. Bank of America can then release your transaction history, claims activity, card details and statement information to the person you name. You can also authorize this individual to file and dispute claims on your accounts. To designate an individual, complete the Authorization to Act/Release Account Information Form.

Affidavit for Power of Attorney

A power of attorney can contact Customer Care to access your health and benefit account information and make transactions on your behalf. To designate a power of attorney send in your legally drafted power of attorney documents along with the Affidavit for Power of Attorney Form.
