Save your receipts

You may be required to provide a receipt or Explanation of Benefits (EOB) to document a qualified expense, so it is important to note the required information for a receipt.

  • Name of person incurring the expense
  • Date of service
  • Amount charged
  • Merchant/Provider name
  • Type of service or prescription number

An EOB or an invoice from your provider are good examples of receipts that are likely to include the necessary information.

Standard cash register receipts typically do not include adequate detail to substantiate a claim because they don't include the name of the person incurring the expense or the type of service.
